We now operate our appointment system via triage system. 

You are able to access the triage form via your NHS App, via the Keats Grove Surgery Website Homepage or via the link below

Online Triage Request (including Appointments)

If it is not possible for you or you are unable to access the triage request these ways, you can still call the surgery or attend in person and our reception team will be happy to complete the online form for you - telephone 01902 731907.

Our triage system is for acute medical conditions, routine appointments and for administrative queries, e.g. fit notes, medications queries etc

Any registered patient under the age of 16 years must be accompanied by a parent / carer.

Chaperone - all patients are entitled to have a chaperone present for any consultation, examination or procedure where they feel one is required. Please ask a member of staff about this and they will be delighted to assist you.

Urgent appointments

All appointments go through our triage system. A GP will assess the triage request  and arrange the most appropriate care for you. 

Cancelling or changing an appointment

If you are no longer able to attend your appointment, or if you wish to change or cancel, please inform reception as soon as possible.  You can cancel by replying “cancel” to your text message reminder, online, or by phone.

Home Visits

It is always better to attend surgery wherever possible.

Patients who are housebound or their illness prevents them from attending surgery should complete an online triage form, which will be assessed by the traiging GP. The GP will determine the appropriate care required which may involve telephone consultation or where needed a home visit.

Enhanced Access Appointments

Contact your GP Practice to book an appointment. Please note, you must be registered to one of our network's practices.
